The following data collection is being done on a voluntary basis.  This is not mandated, but is an important data collection to demonstrate the response of the school nurse in a medical emergency.  Please complete one of the following forms if the emergency medication or treatment listed is used:

Online Immunization School Assessment Survey Information

The New York State Department of Health Immunization Program's Online School Assessment Survey (OSAS) is now available for New York State to use.  The goal is to have at least fifty percent of public school districts and non-public schools submit their annual mandated immunization survey online in the 2008 - 2009 school year, and the remaining districts and non-public schools ready to submit online for the 2009 - 2010 school year.
